Home Modification and Maintenance Services (HMMS) are available to people across the Central Coast to support them to live independently. The services are available for those who have been assessed as eligible through My Aged Care under the Commonwealth Home Support Program, the National Disability Insurance Scheme, the Safe and Supported at Home program or as fee for service. Home Care Package recipients can access these service through their package funds.

 

Our dedicated team of Trades Officer and contractors provide a diverse range of services. They work collaboratively with Client's, Carers, family members and a range of Allied Health Professionals.

 

Coast Community Connections hold membership to HIA (Housing Industry Association), and Home Modifications Australia (MOD.A), the national body that represents home modification providers for older people and people with disabilities. We have an up to date Builders and Contractor licence and relevant insurance requirements.

Our Service has achieved third party verification in meeting the NSW National Disability Standards (September 2014) and the Australian Home Care Standards (April 2015) through Institute for Healthy Communities Australia and has achieved recognition from the Australian Aged Care Quality Agency as meeting all the Australian Home Care Standards (August 2015).
