Our Aged Care team provides support to older people helping them maintain an active, independent lifestyle. This includes providing support directly to clients, carers and their families across the Central Coast.

Our support can be flexible as your needs, circumstances and goals change and of course, within your budget.
Our community connections means we can also link you to the right services and supports if we are unable to deliver these directly to you. Referral services may include allied health and nursing, ensuring you have choice and control.
Coast Community Connections Ageing and Disability Services have remained committed to strive to deliver safe high-quality in-home care support and services for older people and people with a disability to reside across the Central Coast. Our team works collaboratively with Clients and other stakeholders to ensure best practice, identtify Client strengths and goals while promoting and supporting independence.
Funding Sources
Commonwealth Home Support Program (CHSP)
CHSP is an entry-level program that provides subsidised in-home aged care services for people needing a little help.
Home Care Package (HCP)
HCP provides individuals with a monthly allocation of funding to purchase services or equipment.
Privately Funded
Services
While waiting for funded care, or where funding does not apply, we provide experienced, trained care workers at competitive rates.
